There are four levels of hospice care to ensure that patients receive appropriate and individualized support based on their changing needs throughout their end-of-life journey.

  • Continuous Critical Care

Each level offers a different level of intensity and setting, allowing for flexibility in addressing symptom management, caregiver respite, crisis situations, and overall comfort.

Pain and Symptom Management:

Providing comprehensive care and interventions to alleviate physical and emotional discomfort, ensuring optimal comfort and quality of life for patients nearing the end of life.

Skilled Nursing Visits:

Our team of experienced and skilled nurses are scheduled to visit weekly to ensure our patients receive the highest quality of care.

Medications:

All medications are reviewed and 

prescribed by our Medical Director before being delivered to your residence. 

Social Services:

Medical social workers  provide emotional support and counseling to patients and their families, while also assisting with care planning, resource coordination, and advocating for their needs within the healthcare system.​

Volunteer Services:

Trained volunteers may provide companionship, run errands, offer respite for caregivers, or assist with practical tasks to support patients and their families.​

Bereavement Support:

Spiritual Counselors offer assistance and emotional support to family members after the death of a patient. They assist in navigating the grieving process and how to cope with their loss in a healthy way.​

Palliative Care:

Improving the quality of life for our patients facing serious illnesses. We address the physical, emotional, social, and spiritual needs of patients and their families.

Medical Equipment:

We provide medical equipment for our patients while they are in our care. Ex: Wheel chairs, walkers, oxygen equipment, hospital beds, etc. â€‹

Monthly Supplies:

We provide monthly supplies such to our patients which cover: incontinent supplies, personal care, and wound care.​

Dietary Counseling:

Dieticians ensure that patients receive appropriate nourishment tailored to their specific needs and goals, while also addressing any symptoms or challenges related to eating and nutrition.​

Spiritual & Emotional Counseling:

Our Spiritual Counselors offer spiritual care and support to patients and their families based on their individual beliefs and preferences.​

Home Health Aide Assistance:

Home health aides assist with personal hygienic care, such as bathing, dressing, grooming, and more.​

What conditions qualify for hospice care?
