Assisted Living Waiver (ALW) Program

As a Care Coordination Agency (CCA), we genuinely care about meeting the unique placement needs of individuals, their loved ones, and residents. With our wealth of resources and expertise, we strive to ensure a seamless and personalized transition that caters to our patients.

We aim to ensure a secure and prompt transfer of eligible seniors and individuals with disabilities who qualify for Medi-Cal from nursing facilities to community-based residential settings such as Residential Care Facilities (RCF), Adult Residential Care Facilities (ARF), or public subsidized housing. This is made possible through the utilization of ALW services, enabling a smooth transition to a comfortable home-like environment.

At I Care Hospice, we offer the chance for eligible seniors and individuals with disabilities in the community, who are at risk of being institutionalized, to make use of ALW services. This allows for the development of a customized care program that addresses their specific needs while enabling them to stay in a Residential Care Facility (RCF), Adult Residential Care Facility (ARF), or public subsidized housing.

To enhance the overall quality of life for patients by ensuring they receive appropriate care and support in a home-like environment.

Support:

To provide comprehensive support and services to patients, including personal care assistance, medication management, meal preparation, transportation, and social activities.

Choice:

​To offer individuals the option to reside in an assisted living facility rather than a nursing home, based on their preferences and needs.​
